I understand that interest will continue to accrue on unpaid balances during any approved payment plan. I understand that late fees, interest, and costs are not waived or reduced. I understand that failure to make any payment when due will result in immediate default and resumption of all collection and enforcement actions, including lien and legal action, without further notice.

What happens next: The Board will review your request at the next scheduled board meeting. You will be notified of the decision in writing. If approved, you will receive a formal payment plan agreement to sign.
